Illinois Prison Project Legal Clinic, Advice and Consultation Agreement
ACCEPTANCE OF ADVICE AND CONSULTATION SERVICES
I understand that Illinois Prison Project (IPP) is agreeing to provide only legal advice and consultation services to me. I understand that this is NOT an agreement for IPP to represent me or my loved one. The services that IPP is providing to me are limited to advice, referral, and consultation. These services do not constitute or create an agreement to represent me or my loved one in any formal or informal capacity. I understand that IPP is NOT my attorney and is NOT my loved one's attorney. IPP will NOT prepare or file any legal documents on my behalf. IPP will NOT represent me or my loved one at any hearings.
I understand that, if IPP decides to provide full representation in the future, they will sign and ask me to sign a separate full representation agreement (also called a retainer). If that does not happen, the limitations described above will continue to apply (meaning that IPP does not represent me or my loved one). Should IPP learn of a potential conflict of interest, we may decline to speak with you directly.
The legal advice that IPP provides me will be based on my statements, expectations, and information available to IPP, as well as how IPP understands how the law applies to my case. For that reason, I understand that it is important that I am as truthful as I can be and that I assist IPP in obtaining any documents that they might need in order to help me.
I understand that IPP cannot guarantee any particular outcome regarding what will happen with my or my loved one's case. That means that IPP cannot promise that the case will be successful.
Although information I share with IPP will be treated as confidential, participation in this legal clinic does not create an attorney-client relationship in any way.
I have read and understand this agreement.

ACEPTACIÓN DE SERVICIOS DE CONSEJO Y CONSULTA
Entiendo que Illinois Prison Project (IPP) está de acuerdo a presentarme consejo legal y servicios de consultación. Entiendo que esto NO es un acuerdo con IPP para representarme a mí o a mi familiar. Los servicios que me van a presentar son limitados a dar información, referencia, y consulta nomas. Éstos servicios no crean un acuerdo de representar a mí o a mi familiar en cualquiera manera formal o informal. Entiendo que IPP no es mi abogado ni es el abogado de mi familiar. IPP NO preparará ni presentará ningún documento legal en mi nombre. IPP NO me representará a mí ni a mi familiar en ninguna audiencia legal.
Entiendo que si IPP decide a representarme oficialmente en el futuro, que IPP va a firmar un acuerdo separado y completo (también llamado un anticipo), y van a pedir que yo lo firme también. Si eso no sucede, las limitaciones descritas antes seguirán aplicándose (lo que significa que IPP no me representa a mí ni a mi familiar). Si IPP se enteraría de un posible conflicto de intereses, podemos negar a hablar con usted directamente.
La información legal que IPP me presenta, va a ser basada en mis declaraciones, esperanzas, información que IPP puede localizar, también basado en cómo IPP entiende que la ley aplica a mi caso. Por eso, entiendo que es importante que yo diga la verdad completa, y que yo ayude a IPP en obtener todos los documentos que ellos necesitarán para ayudarme.
Entiendo que IPP no puede garantizar ningún resultado en particular con respecto a lo que sucederá con mi caso o el de mi familiar. Eso significa que IPP no puede prometer que el caso tendrá éxito.
Aunque IPP tratará la información que comparto con ellos en una manera confidencial, la participación en esta clínica legal no crea una relación profesional de abogado-cliente de ninguna manera.
